The Existential Dispatch, Issue No. 29
This issue turns toward the encounter: the irreducible human relation that makes care more than the delivery of a service. Thinker of the issue: Ludwig Binswanger.
A Word from the Founding Director
This issue turns toward the encounter: the irreducible human relation that makes care more than the delivery of a service. Recent work on palliative care and mental disorder, workforce conditions, grief in film, and a Canadian trial of psilocybin microdosing all raise the same question: how do we protect communication from becoming merely instrumental? Existential inquiry begins where a person is still more than the problem a system has been asked to solve.
Existential Thinker of the Issue
Ludwig Binswanger
Ludwig Binswanger (1881–1966), a Swiss psychiatrist and pioneer of existential analysis, brought phenomenological attention to the patient’s lived world. His account of communication is quietly radical: what matters most in a clinical relationship cannot be manufactured by technique alone. Trust is not a procedure, but a way of being with another person.
“It lies, as the gift of all true communication, beyond intention, means and ends, cause and effect.”
Ludwig Binswanger quotation source: citeseerx.ist.psu.edu
Clinically, this is a useful guard against confusing communication with information transfer. A careful question, an explanation, or a handover can still fail if the patient experiences no genuine meeting. Binswanger invites us to ask what makes presence possible: time, continuity, humility, attention to the person’s world, and the willingness to let the encounter exceed our planned aims.

Palliative care for people living with mental disorder
A review published in Frontiers in Psychiatry on August 4 synthesises 11 qualitative studies of healthcare professionals providing palliative care to patients with mental disorders. Its themes include perceptions of palliative care, multi-level barriers, and possible improvements—an important reminder that psychiatric diagnosis should not make finitude, suffering, or the need for comfort disappear from view. frontiersin.org

Bill Evans, grief, and the music that remains
A Guardian feature on Grant Gee’s film about jazz pianist Bill Evans follows the musician through the aftermath of bassist Scott LaFaro’s death. Its focus on grief, addiction, artistic work, and the moment “when the music stops” offers a cultural meditation on how a life continues when its defining relationship has been broken. theguardian.com

The workforce is part of cancer care
A July 24 joint statement from the International Council of Nurses, the International Society of Nurses in Cancer Care, and the International Children’s Palliative Care Network responds to the WHO cancer report by calling for investment in nurses and multidisciplinary teams, action on shortages and burnout, and a move toward people-centred, equitable care across prevention, treatment, survivorship, rehabilitation, and palliation. Equity is not only a question of access to therapies; it is also a question of whether there is a person there to accompany us. icn.ch

PSYCHED-PAL moves into Phase 3 recruitment
The Bruyère Health Research Institute’s PSYCHED-PAL trial was listed as active and recruiting on July 8. Across seven sites, it plans to enrol 120 adults with advanced illness and psychological distress to compare two weeks of low-dose psilocybin with placebo; the existential question is not whether a molecule can remove finitude, but whether carefully governed treatment can help people live with what cannot be removed. centerwatch.com
To Close: An Existential Joke
Why did the existential psychiatrist bring a stopwatch to the session?
He wanted to measure the moment—but the encounter refused to be quantified.
